Dates of Completed Hours Deadlines for Submitting Forms

Summer hours Aug. 31; 4:00 p.m.

Aug. 31 – Jan. 10 Jan. 11; 4:00 p.m.

Jan. 11 – Mar. 19 Mar. 20; 4:00 p.m.

Mar. 20 – Apr. 29 Apr. 30; 4:00 p.m.

Please note the dates of your volunteer service. Accumulated hours during the dates in the first column are due by the deadlines given in the second column. By turning in forms in increments, accuracy of hours, as well as approval of service provided, can be assured before the last deadline for the year. If you haven’t reached 25 hours or have done a service that cannot be counted, then it will be discovered so you can notified with time to complete the hours necessary to meet the minimum requirements.

All forms are due only to Ms. Loftin in room A117. Please keep for yourself a copy of all forms you turn in. Your red PIB notebook must have a divider labeled “CAS” – for copies, directions for proposals, log sheets, additional CAS forms, etc. If additional forms are needed, either go to the Lee IB website or to Ms. Loftin for more.
