Parents/Guardians please complete, sign and send both the campus permission form for our web site and the internet release form to us.  Both forms are required and inform us about publishing your child's picture, name and information on our Lee web page. Please return both forms to your child's Academy/School principal. The secretary for each Academy/School keeps each form on file.
