Five doses of any combination DTaP/DTP unless 4th
dose was given on or after 4th birthday.
Students 7 years or older:
Three doses of any combination DTP/DTaP/DT/Td vaccine (pertussis vaccine
isnot required.)
One dose of Td required ten years after last dose of DTP/DTaP/DT. |

Polio (IPV) |
Four doses unless the 3rd was on or after 4th
birthday. |

Measles, Mumps, Rubella (MMR) |
Two doses of a measles-containing vaccine with
the first dose on or after the first birthday; second dose by age 5 or entry
into kindergarten. |

Hepatitis B |
Three doses are required for the
following grades in the follow school years:
2006-07 K-12 |

Varicella |
One dose on or after 1st birthday
for the following grades:
2006-07 K-12 (Two doses if vaccine given at 13 years of age or older.) |

Students under 5 years of age
One dose on or after 15 months of age, unless a primary series and booster
have been completed. |
All
pre-kindergarten, Head Start and other early childhood students will need to
provide records documenting that they are on-schedule for the hepatitis A
and pneumococcal conjugate (PCV7) vaccines. Students in certain high-risk
groups such as sickle cell disease, cochlear implants, chronic heart or lung
disease, or immunocompromising conditions will also be required to have the
pneumococcal polysaccharide vaccine (PPV23).
